Comments:
An unannounced monitoring visit and facility tour was conducted from 10:50 am - 12:10pm. At the time of inspectors' arrival there were 30 infant-preschool aged children in care with 6 staff members. Children were observed participating in outdoor play, lunch, transitioning between activities, toileting and hand washing. A sample of 6 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are detailed in the Violation notice and were discussed with the staff in charge at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review it was determined that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The record for child #5(DOB 1/26/17) did not contain updated immunization documentation.

Plan of Correction: The Director will request updated documentation of immunization from the parent at pick up.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. In the Jellyfish room there was broken vinyl tiles on the bathroom floor. 2. In the Starfish room there were 4 long splits in the vinyl tiles on the bathroom floor. 3. Outside on the main playground there was peeling paint on the 6 play structures and fencing. There was also a rotting board with splintering ends on one of the wooden play structures. 4. These areas were all accessible to children in care during this inspection and presented potential injury hazards.

Plan of Correction: The center will repair the bathroom floors and splintering wood. The play structures and fencing will be painted when the weather permits.
