Comments:
A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. Licensing Inspector arrived at 9:45am and departed at 2:15pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 79 children in care with 16 staff present. Children were observed playing with toys, during circle time and being read a story. Lunch was also observed. A sample of 5 staff and 8 children's records were reviewed in addition to 2 medications. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a record review and interview with the assistant director, the vendor failed to ensure there was a written care for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #6 was listed on the vendor's "Allergy & Dietary Preference," list as having a food allergy indicating a high risk for a severe reaction. Child #7 and #8 were also listed as having food allergies. Neither Child #6, #7 or #8 had a written allergy plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The director will contact each parent and request an allergy plan for each.
