Technical Assistance:
Discussed Medication Authorization forms and discussed verification of experience. Program must submit Background documentation within 10 days.

Comments:
An unannounced inspection was conducted from 940- 1130 AM. There were 94 children (ages infants through 5 years) present today and the school age extended program was not checked. Classes, outside program, equipment, ratios, and special care were all observed. The Inspector also reviewed files, medication, and attendance records. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, it was determined the center did not obtain a current TB Screening report for 1 staff member. Evidence: 1. Staff 1 was hired and began employment in December of 2017. The TB Screening report is dated from April, 2016 which is over 12 months prior to employment.

Plan of Correction: We will request that a TB Screening is obtained.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not keep areas and equipment of the center, inside and outside, maintained in a safe manner. Evidence: 1. The Inspector observed chipping paint on the walls and observed chipping paint on the lower portion of a wall, in the older infant room. 2. Inspector observed the large playground and saw that the rubber mulch did not completely cover the fabric rolls in many areas. One area where this tripping hazard was noted was close to a round, metal ground drain cover.

Plan of Correction: We spoke to facilities about the walls and will follow up with them regarding the painting. We will go out and redistribute the mulch and will make sure each day that tripping hazards are not repeated.

Standard #: 22VAC40-185-430-D
Description: Based on observation, the center did not ensure that a climbing structure on the toddler playground is located over resilient surfacing.

Plan of Correction: We will keep children from using that piece of equipment until resilient surfacing is ordered and placed onto the playground.

Standard #: 63.2-1721.1-B-3
Description: Based on record review, the center did not obtain a CPS check in another state for staff #, who started in December, 2018. Evidence: 1. Staff 1 has lived in another state in the last 8 months and the center did not obtain a Background check in that state.

Plan of Correction: We will send out a CPS check to that state and will check all new staff to ensure proper searches are conducted.
