Comments:
The following inspection took place between the hours of 10 am through 2 pm. Today, I met with the Staff of the ImagiNation Learning Center located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 100 children with 22 classroom staff; good ratios in all 9 rooms. A total of 10 children?s files and 5 staff files were reviewed. The children were engaged in the following: outdoor activities to include large motor skills on playground, walks, outings for the younger children in push-buggies; story time, art, diapering, infant feeding, hand washing, music, bathroom breaks, leaning Spanish, lunch, nap time, and other organized activities. If you should have any future questions, please call. Thank you. Charlie Perkins, Licensing Inspector (703) 309-3963

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review, 1 of 10 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence - 1. On the date of inspection (02/21/2018), the current immunization page on file for Child #6 was dated - 02/01/2017. 2. Based on the information on file, Child #6 was under the age of two years on the date of inspection.

Plan of Correction: An updated immunization page shall be obtained from the parent of Child #6.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area and equipment within the center was not maintained in a safe and/or operable condition. Evidence - 1. On the date of inspection (02/21/2018), the back wall area, below the window, located within the Panda Room shows signs of heavy wear and paint that has chipped off. It appears that the paint may have been removed when pictures were lifted off its surface and the adhesive tape pulled the paint up with it. 2. The blinds within the Zebra Room were bent and broken in several places. On one blind an entire slat appears to be hanging away from the others.

Plan of Correction: These blinds shall either be removed or replaced.
