Comments:
An unannounced monitoring inspection was conducted on February 15, 2018, between the hours of approximately 10:15 a.m. and 3:00 p.m. Sixteen (16) children were enrolled and fifteen (15) children were in the care of two/three staff members, in one group, during the on site inspection. Five children's records were reviewed and five staff records were reviewed and/or updated. No medications were on site/administered on this date. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan, emergency telephone numbers, snack menu and classroom schedule), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill and shelter-in-place drill records) were reviewed. The daily schedule accurately reflected the activities provided. Lunch was provided by the children's parents. Program observation was completed. Activities observed on this date included: outdoor play; indoor play; diapering, restroom and handwashing procedures; lunch; stories; and naptime. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Children's artwork was displayed. Previously cited violations were reviewed for compliance (8/28/2017 inspection date); two repeat violations were cited. Exceptions to compliance were noted on the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on review and/or update of four staff records and discussion with staff #1 on February 15, 2018, the program failed to ensure one staff member submitted the results of a follow-up tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing. Evidence: 1. Update of staff #3's record, on this date, determined that the results of an updated TB exam/screening had not been obtained. The date of the initial TB screening was 11/01/2013. 2. Staff #1 confirmed the date of the initial TB screening and lack of an updated exam/screening.

Plan of Correction: An updated TB exam/screening will be obtained.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review and/or update of four staff records and discussion with staff #1 on February 15, 2018, the program failed to obtain updated criminal history record checks for two staff members. Evidence: 1. Update of staff #1's record, on this date, determined that a current criminal history record check was not on file. 2. Documentation provided by staff #1 on February 16, 2018 determined that an updated criminal history record check was obtained September 18, 2017 and the previous criminal history record check was dated December 19, 2013. 3. Update of staff #3's record, on this date, determined that a current criminal history record check was not on file (clearance on file dated December 13, 2013). 4. Staff #3 completed the fingerprinting process (clearance dated February 6, 2018). 3. Staff #1 confirmed the dates of the criminal history record checks and the fingerprinting clearance.

Plan of Correction: In the future, background clearances will be obtained in a timely manner.
