22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:42 am-2:15pm. At the time of entrance, 93 children were in care with 21 staff members present. Children were observed during circle time, snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center, outside were not maintained in a safe and operable condition. Evidence: 1. The playground gate was difficult to open and shut. 2. At the bottom of the rock wall, there is a half circle stake sticking out of the ground. Plan of Correction: I will report it to the maintenance division to have repaired by tommorrow.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Two's classroom bathroom, there were 3 clorox wipe containers in a higher unlocked open cabinet above the sink and one clorox wipe containers in a closed unlocked cabinet that had a hook eye. 2. In the Three's classroom, there were 5 disinfecting sprays and 1 clorox wipe containers in a higher unlocked cabinet with a hook eye. 3. In the Fours classroom, the bathroom in the hall that the children use had an unlocked cabinet with 3 all purpose cleaners and 3 clorox disinfecting wipe containers. Plan of Correction: I will replace the hooks with a lock or replace with child safety sliders to go over knobs. The cleaners were moved to a locked closet for the time being.
Standard #: 22VAC40-185-430-C Description: Based on observation, play equipment used by children did not meet all the requirements. Evidence: 1. The blue base swing set had 3 swings with S-hooks that were open more than the thickness of a penny. 2. The red base swing set had 2 swings withS-hooks that were open more than the thickness of a penny. 3. The red boat play structure had one stake protruding out of the ground. Plan of Correction: This will be fixed today.
Standard #: 22VAC40-185-510-J Description: Based on observation, all medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. When checking medication in the four year old room, Child A's medication was inside a tote bag in a cabinet in a safety sack, but it was not zipped closed. Plan of Correction: The teacher zipped it immediately.
Standard #: 22VAC40-185-550-D Description: Based on review and interview with the Director, the center did not implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: 1. The center implemented one shelter in place drill on 5/18/2017, but did not implement a 2nd shelter in place drill in 2017. Plan of Correction: I will make sure two shelter in place drills are implemented this year.
