During the inspection, the licensing representative discussed with staff requirements for maintaining records.
A Subsidy Health and Safety inspection was conducted on 2/23/18 to review supplemental health and safety requirements. Today's inspection took place from 3:40pm-5:50pm. A total of 44 children were in the care of 4 staff present. 5 children's records, 4 staff records, injury reports, medications, evacuation/shelter in place drills and emergency supplies were reviewed. Children were observed during homework time, handwashing, snack, and while playing in the gym. Gold fish, milk and applesauce were served for lunch today. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Standard #: VENDSUB-000-009-2 Description: Based on a review of injury reports, the center did not inform the licensing representative of serious injuries of children in their care within two business days. Evidence: The following incidents were not reported to the department within 2 business days: 1. An injury involving Child #6 on 1/9/18 which resulted in a sprained ankle. 2. An injury involving Child #7 on 11/6/17 which resulted in a "severe cut" that received 14 stitches 3. An injury recorded on 10/12/17 involving Child #8 who received a bump on the head which resulted in the parent transporting the child to the hospital. Plan of Correction: Child #7 was already released to authorized pickup at the of injury; Though child was not in the care of SACC at the time of injury an injury report was written as a backup; For the other incidents, SACC program will notify the department; Will clarify policies with Regional Supervisor.
Standard #: VENDSUB-000-012-B Description: Based on a review of records and discussion, documentation of the completion of required the staff training within 90 days of employment was not available at the time of inspection. Evidence: Documentation of the completion of the Virginia Pre-service Training for Child Care Staff was not available for Staff #4 [date of hire: 2016] at the time of inspection. Plan of Correction: We will make every substitute has a copy of the required training with them.
Standard #: VENDSUB-000-012-E-1 Description: Based on a review of records and discussion, documentation of the completion of the required staff training within 30 days of employment was not available at the time of inspection. Evidence: Documentation of current certification in cardiopulmonary resuscitation was not available for Staff #4 - who reported being hired in 2016. Plan of Correction: We will make every substitute has a copy of the required training with them.
Standard #: VENDSUB-000-012-E-2 Description: Based on a review of records and discussion, documentation of the completion of the required staff training within 30 days of employment was not available at the time of inspection. Evidence: Documentation of completion of the Virginia Preservice Training for Child Care Staff current first aid certification was not available for Staff #4 at the time of inspection: Staff #4 reported that they were hired in 2016. Plan of Correction: We will make every substitute has a copy of the required training with them.
