The licensing inspector discussed separate medications for siblings with the director.
An unannounced renewal inspection was conducted today from approximately 10am-12:50pm. There were 91 children in care directly supervised by 15staff. A sample of 9 children?s and 6 staff records were reviewed. The children were observed napping, playing on the playground, exercising, watching movies, and doing puzzles. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-185-160-C Description: Based on record review, a tuberculosis exam was not repeated at least every two years. Evidence: Staff A was did not have a repeated tuberculosis test. The last screening was conducted 12/1/2015 and expired 12/1/2017. Plan of Correction: She will get a screening tomorrow.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: 1 of 9 children's records reviewed were incomplete. 1) Child A was missing work address of 1 of 2 parents. Plan of Correction: Both parents work from home.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: An aerosol can of disinfectant solution was found on the counter in the "Butterflies" classroom. Plan of Correction: They will lock it .
Standard #: 22VAC40-185-320-B Description: Based on observation, each restroom was not maintained with the required contents. Evidence: The restroom in the "Teddy Bear's" classroom was missing toilet paper. Plan of Correction: Completed during inspection.
Standard #: 22VAC40-185-550-M Description: Based on record review, the records of minor injury logs were incomplete. Evidence: Of 16 minor injury records reviewed : 1) 6 were missing the method in which parents were notified of the injury. 2) 4 were missing the time in which parents were notified of the injury. Plan of Correction: I will discuss the injury logs procedure with the staff.
