VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
VENDSUB-000 SPECIAL CARE PROVISIONS AND EMERGENCIES

Comments:
Licensing inspector arrived at 7:50 am and departed at 2:40pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of arrival was thirty - eight children in care with 6 staff . Additional staff and child arrived during the inspection. The Licensing Inspector observed children arriving , coloring, reading , freely playing, eating lunch and breakfast, diapering, bathroom break, and story time. The sample size consisted of 8 children records and 7 staff records. Areas of non-compliance was reviewed with the director.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on observation, the vendor did not ensure a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence : Child #7 had food allergy diagnosed by doctor. Child #7 did not have instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Management and staff will all be retrained on or before 3/23/18 on all required allergy plan requirements. All files will be update on or before 3/23/18.

Standard #: VENDSUB-000-012-E
Description: Based on interview and 2 of 7 records reviewed, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment . Evidence: During the subsidy inspection conducted on February 23,2018, staff #1 ( Date of hire 10/11/2016) and staff #7 ( Date of hire 03/27/17) did not have documentation of completing CPR and First aid training .

Plan of Correction: All staff without current CPR and FA are scheduled to attend a class on 3/8/18. Signed cards verify attendance will be forwarded to the DM to verify. Moving forward all new hires will have to submit proof of training to the DM prior to the end of their first 30 days of employment.

Standard #: VENDSUB-000-012-E-1
Description: Based on interview and 1 of 7 records reviewed, the vendor did not ensure staff providing care to children had a current certification in CPR and First Aid. Evidence: During the subsidy inspection conducted on February 23, 2018, staff#6 did not have a current CPR and First Aid certification. Staff #6 CPR and First Aid certification expired on 9 /15/2017.

Plan of Correction: All staff without current CPR and FA are scheduled to attend a class on 3/8/18. Signed cards verify attendance will be forwarded to the DM to verify. Moving forward all new hires will have to submit proof of training to the DM prior to the end of their first 30 days of employment.

Standard #: VENDSUB-000-031-A
Description: Based on and interview and review of procedures, the vendor did not have emergency preparedness plan with reunification for evacuation, shelter in place and lockdown. Evidence: During the subsidy inspection conducted on February 23, 2018, the vendor did not have procedures addressing reuniting children with parents or authorized person designated by the parent to pick up the child for evacuation , shelter in place and lock down. The word reuniting or reunification was not used in the plans.

Plan of Correction: Emergency preparedness plan was updated and resubmitted by the DM on 3/6/18. All staff were retrained that same day and parents will all receive written updates on or before 3/9/18. DM will review plan annually or as needed with center management for any changes.
