An unannounced monitoring inspection was conducted today 2/26/2018. The inspection began at 8:30 A.M. and concluded at 1:45pm. There were 63 children present with 12 staff members and the Program Director were also present during the inspection. During the time of the inspection all eight classrooms were observed, infant care was observed, diapering was observed, hand washing was observed, morning activities, arrival into the program, breakfast, art activities were observed, lunch time was observed, nap and emergency supplies were viewed. Discussion with the program director about: background requirements and talking with home office about the requirements, and ensuring that adverse reactions are completed on the topical authorization form . The plan of correction is due in the licensing office on Friday March 2, 2018
Standard #: 22VAC40-185-60-A Description: Based on review of six children's records the center failed to ensure that all records were complete. Evidence: 1. The record of child # 2 did not have parent employment information for both parents. 2. The record of child # 3 did not have father's employment information. Plan of Correction: The director has already completed the missing information for child # 3 and # 4.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to ensure that all areas and equipment, inside and outside was maintained in a clean, safe and operable condition. Evidence: 1. Observed in the Purple Ponies room, the play wooden kitchen had two doors that were no longer attached and posed a pinching hazard to the children. Plan of Correction: The director removed the kitchen set out on the day of the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all chemicals were locked. Evidence: 1. Observed the laundry room door opened. Sitting on the floor right next to the door was several containers of laundry detergent. Plan of Correction: The director spoke with the cook to remind her to close and lock the laundry door and a note has been added to the door.
Standard #: 22VAC40-185-280-G Description: Based on observation, the center failed to ensure that all substitute containers clearly indicated their contents. Evidence: 1. Observed in the Purple Ponies room there were two spray bottles that clearly did not indicate their contents. Plan of Correction: During the inspection the teacher labeled the spray bottles, all bottles will be monitored.
Standard #: 22VAC40-185-520-A Description: Based on observation, the center failed to ensure that all over the counter skin products were used in accordance with the manufacturer's recommendation. The products shall not be kept or used beyond the expiration date. Evidence: 1. Observed in the infant room, a diaper cream that expired on 8/2016. Plan of Correction: During the inspection the diaper cream was discarded.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center failed to ensure that all requirements per the standard were met for using diaper cream/ointment. Evidence: 1. Observed a diaper cream in the infant room that was not labeled with the owner's name. Plan of Correction: During the inspection the diaper cream was discarded.
