Discussed the recent communication regarding fingerprint based background checks for child day programs.
The licensing inspector conducted an unannounced renewal inspection Wednesday February 21, 2018 from10:30am to 2:20pm. The children were observed in "open time" in which they are self directed and choose which learning classroom they would like to work in. They were observed shooting basketball hoops in the gross motor room, playing in the dramatic play room and playing outside on the playground. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/30/2018 Last shelter-in-place drills: 11/14/2017 Today, the following child to staff ratios were observed: Pink Room (Two's) -11:2 Outside Group (Preschool) -11:2 Gross Motor Development (Preschool)-9:1 Dramatic Play (Preschool)-9:1 Transportation (Preschool) -10:1 Fine Motor Group (Preschool) -15:2 Art Group (Preschool)-9:2 Science Group (Preschool)-12:2 Construction Group (Preschool) -12:2 Library Group (Preschool)-9:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records on 02/21/2018 , the center did not ensure that one of ten records contained a physical examination before the child's attendance; or within one month after attendance. EVIDENCE: The record of Child #7, whose first day of attendance at the center was 09/05/2017, did not contain a physical examination. Plan of Correction: Will make sure parent turns in complete physical with shot record.
Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 02/21/2018, the center did not ensure that three of eight records reviewed contained documentation of a negative tuberculosis (tb) screening no later than 21 days after employment. EVIDENCE: The record of Staff #1, who started at the center on 07/01/2016, does not contain documentation of a tb screening. Plan of Correction: Will have teacher get from doctor. She did tb on 12/6/2016.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records on 02/21/2018, the center did not ensure that two of ten record reviewed contained an address for two designated people to call in an emergency if a parent cannot be reached. EVIDENCE: The records of Child #1 and Child #3 did not have an address listed for two emergency contacts. Plan of Correction: Will call parents tomorrow and get address. In the future will make sure forms are completed before they are turned in.
Standard #: 22VAC40-185-70-A Description: Based on a review of staff records on 02/21/2017, the center did not ensure that three of ten records reviewed contained the required information. EVIDENCE: 1. The record of Staff #10 did not contain the address of the person to be notified in an emergency. 2. The record of Staff #5 did not contain documentation of 2 or more references. 3. The record of Staff #4 did not contain the name, address and telephone number of a person to be notified in an emergency. 4. The record of Staff #10 did not contain information about any health problems which may interfere with fulfilling the job responsibilities. Plan of Correction: Will get references letters. Will have staff fill out missing address and health information.
Standard #: 22VAC40-185-260-B Description: Based on a review of documentation on 02/21/2018, the center did not ensure that approval from the health department was provided annually. EVIDENCE: The most recent health inspection was dated 01/20/2017. Plan of Correction: The inspector is scheduled to come 03/01/2018.
Standard #: 22VAC40-185-330-B Description: Based on observation on 02/21/2018, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. EVIDENCE: Two of the two depth samples taken of the mulch under and around the sliding board on the playground measured less than six inches. Plan of Correction: Ordered mulch today and it will be here tomorrow.
Standard #: 22VAC40-191-40-D-1-A Description: Based on a review of records on 02/21/2018, the center did not ensure that a new person designated as an agent has a sworn statement or affirmation, search of central registry and criminal history record check before the end of 30 days after the change. EVIDENCE: 1. The record of Board Officer #1 is missing a completed search of central registry. 2. The record of Board Officer #4 is missing a completed search of central registry and criminal history record check. Plan of Correction: Will have officers do background checks tomorrow, 02/22/2018
