22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 11:00am-3:30pm. There were 27 children (infants-4yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, eating lunch, and napping. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the center did not maintain complete children's records. Evidence: The record for child#1 does not contain a house or street number for emergency contact#2, the file is also missing the father's work phone number. Child#2's record is missing the parent's place of employment and the phone number. Child#3's record is missing the father's place of employment and the phone number. Child#5's record is missing the father's work phone number. Plan of Correction: We will contact the parents today and have them complete an emergency release form.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff records, the center did not maintain complete staff records. Evidence: Staff#4's record is missing 1 of 2 references required before the first day of employment. The staff member was hired on 4/20/2017. Staff#5's record is missing 2 references required before the first day of employment. The staff member was hired on 4/24/2017. Plan of Correction: We will have home office send us the employee's references.
Standard #: 22VAC40-185-550-D Description: Based on review of the emergency response drills record, the center did not conduct a monthly emergency evacuation drill. Evidence: Documentation of emergency evacuation drills were missing for the months of November and December. Plan of Correction: We'' follow our health and safety calendar to ensure emergency evacuation drills are conducted monthly.
Standard #: 22VAC40-185-560-G Description: Based on observation, it was determined that not all food brought from home was dated and labeled in a way that identifies the owner. Evidence: Two bottles of Pediasure were observed in the Discovery classroom, the bottles were not labeled with the owner's name or date. Plan of Correction: We will remind parents to label and date all food brought from home.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center does not have documentation of an out-of-state central registry check for a staff member that resided outside the state of Virginia within the past 5 years. Evidence: Staff#1 resided outside the state of Virginia within the past 5 years, the center does not have documentation that an out-of-state central registry check was requested. Plan of Correction: We will continue to follow up with out-of-state agencies to obtain the out-of-state record.
