Comments:
An unannounced inspection was conducted on 2/20/18 from 11:15am until 12:30pm and a follow-up visit was conducted on 2/21/18 from 12:50pm until 1:00pm in response to a complaint received by the Eastern Regional Office on 5/20/18 regarding physical plant concerns, resulting in a child being injured while at the center. The area where the incident occurred was observed, center documents were reviewed and staff were interviewed regarding the incident. Based on information gathered during the investigation this complaint is determined to be valid. One violation was cited in the area of physical plant. An additional violation not related to the complaint was found in the area of administration. Both violations are identified on the violation notice and were discussed with the center director during the exit interview.

Description: Based on record review and interview, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunization record for child #1, age 22 months, is dated 3/8/17. Additional immunizations should have been obtained in September 2017. 2. The center director confirmed that the record for child #1 does not contain up to date immunizations.

Plan of Correction: The center director will request a copy of the most recent immunization record from the parent today at pickup.

Complaint related: Yes
Description: Based on observation and interview, the center did not ensure that areas of the center, inside and outside, shall be maintained in a safe condition. Evidence: 1. On 2/16/18, child #1, age 22 months, was playing in the fenced in play area located at the rear of the center. Child #1's foot got caught in a broken section of the concrete sidewalk causing child #1 to fall and hit her face on the sidewalk. Child #1 sustained an abrasion to the left side of her face as a result of the fall. The child's parent and staff #1 confirmed that the child fell as a result of tripping on the broken section of the sidewalk. 2. The licensing inspector observed a section of the sidewalk located outside the infant classroom door where the outer edge is broken and missing. The section of sidewalk where the concrete is missing measures approximately 5 inches by 2 inches. 3. Staff #1 (toddler teacher) stated the hole in the sidewalk had been there since at least October 2017. 4. Staff #2 (assistant director) stated she did not know how long the hole had been there but that it looked like it had been there "for some time."

Plan of Correction: The center director will place a work order today for the sidewalk to be fixed.
