A joint investigation was conducted regarding a complaint of physical abuse with the director and a representative from the local agency on January 24, 2017 from 8:30 AM to 10:30 AM. An exit meeting was conducted with the director on February 13, 2018. Staff interviews and classroom observations were conducted. One child?s record, two staff records, and documentation were reviewed. Staff interviews were also conducted on January 25, 2018 and February 1, 2018. The complaint regarding physical abuse was determined to be valid. Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on staff interview, a staff used the forbidden action of physical punishment. Evidence: Staff A stated that she ?tapped? a nineteen month old child on the hand after being hit in the face by the child. Staff B stated that she observed staff A ?hit? a child on the hand. Staff B stated that the child cried and his hand was red after the physical contact. Plan of Correction: The staff member involved received a written reprimand and required to complete 8 hours of training on the classroom management and approaches and tactics for handling children with difficult or challenging behavior.
Standard #: 63.2-1509-A Complaint related: Yes Description: Based on interviews, the center failed to contact the Department of Social Services to report an allegation of suspected child abuse or neglect. Evidence: On January 18, 2018, the center received a verbal report from a staff member that child abuse may have occurred at the facility. The facility did not contact the Department of Social Services to report the allegation as mandated. Plan of Correction: The staff member involved received a written reprimand and retrained on the requirements and expectations for mandated reporters.
