Discussed with the Director: Playground has been placed off limits due to blown away mulch after the weekend storm; Also discussed AuviQ training available on line.

Comments:
An unannounced monitoring inspection was conducted beginning at 10:00 and ending at 1:15. There were 56 children present, ages 3 months through 5 years of age. Additional school age children will attend after school. Children were observed celebrating Dr. Suess's birthday, singing as a group, playing outside and having lunch. Reviewed 4 staff files - complete Reviewed 9 children's records - see violation notice Reviewed emergency drills - complete and current as are Health and Fire Inspections Reviewed ccident reports - see violation notice. Standard #: 22VAC40-185-60-A
Description: Based on a review of 9 children's records, it was determined that 2 were not complete. Evidence: The record for Child #1 did not have a physical and shot record. Child was enrolled on 9/10/18; Child #2 had no street address for emergency contacts.

Plan of Correction: The Director will notify parents today. All files will be reviewed.

Standard #: 22VAC40-185-550-M
Description: Based on a review of the accident reports from one classroom, it was determined that reports were not complete. Evidence: The majority of the accident reports on file from the Two year old classroom did not have documentation of the tike taht parents were notified of the accident/injury.

Plan of Correction: Staff will be retrained.
