Comments:
A Subsidy Health and Safety inspection was conducted on 03/06/18 to review supplemental health and safety requirements. There were 57 children in direct care of 15 staff. The children were observed playing outside, eating lunch and preparing for nap time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on record review, written allergy plans were not in place for children with food allergies. Evidence: Upon review of the allergy list, children were listed with food allergies. Allergy plans were not obtained from the children's physicians to indicate steps to be taken in case there was an allergic reaction.

Plan of Correction: I will ask the parents to obtain the allergy plans from the physicians to be placed in their records.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review, staff did not have current certification in cardiopulmonary resuscitation (CPR). Evidence: Staff # 1, 3, 4, 5 did not have current CPR certifications.

Plan of Correction: We will make sure that they get the required CPR certification.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review, staff did not have current certification in First Aid. Evidence: Staff # 1, 3, 4, 5 did not have current FIrst Aid certifications.

Plan of Correction: We will make sure that all staff has current first aid certifications.

Standard #: VENDSUB-000-031-A
Description: Based on record review the center did not have a written emergency preparedness plan for a lockdown. Evidence: Two binders were reviewed the shelter in place and fire evacuation procedures. There was no written lockdown plan in place.

Plan of Correction: I will create a lockdown plan and add it to the emergency preparedness plan.

Standard #: VENDSUB-000-032-A
Description: Based on records review and staff interview, annual lockdown drills had not been practiced. Evidence: 1. The emergency drill records did not have any documentation of a lockdown drill practiced within the last year. 2. Center director stated that the lockdown drill has not been practiced.

Plan of Correction: We will conduct a lockdown drill and document in our log.
