22VAC40-185 STAFFING AND SUPERVISION.

Comments:
An unannounced inspection was initiated in the facility on February 23, 2018, regarding staffing and supervision. The inspection included observations, a review of center documentation and interviews with staff members . Based on information obtained during the inspection the preponderance of evidence does support the allegation related to staffing and supervision therefore the complaint is determined to be VALID. Areas of non compliance are noted on the violation notice.

Complaint related: Yes
Description: Based on observation, attendance record review and staff interviews , the licensee did not ensure that for infants, there was one staff member for every four children. Evidence: During a complaint inspection on February 23, 2018 , the licensing inspectors observed and reviewed head count forms in the infant classroom from ages 0-16 months. The center did not comply with the 1:4 ratio. On February 23, 2018 in the infant classroom , two staff were with 9 children from 7:50am- 8:17am. Two staff were with 10 children 8:18am -9:00am. A third staff member arrived at 9:00 am. The infant classroom was not in ratio for one hour and ten minutes. Staff interviewed confirmed the center is not complying with staff to children ratios.

Plan of Correction: Management is in the process of interviewing and hiring new staff. Director is to notify DM if she is unable to make daily ratios, for assistance. We have two support managers assisting with scheduling and hiring to ensure that staffing requirements are met daily as of 3/5/18. DM will be doing weekly monitoring visits to ensure compliance

Description: Based on observation, attendance record review and staff interviews , the licensee did not ensure that for 16 months to two year olds, there was one staff member for every five children. Evidence: During a complaint inspection on February 23, 2018 , the licensing inspector observed and reviewed head count forms in the toddler classroom from ages 16-24 months. The center did not comply with 1:5 ratio. On February 23, 2018, in the toddler classroom, there was 1 staff with 6 children from 8:19am-8:29am. One staff was with 7 children from 8:29am-9:40am. A second staff member arrived at 9:40 am. The toddler classroom was not in ratio for one hour and 21 minutes. Staff interviewed confirmed the center is not complying with ratio.

Description: Based on observation, attendance record review and staff interviews , the licensee did not ensure that for 3 year old through 5 years of age there was one staff member for every 10 children. Evidence: During a complaint inspection on February 23, 2018 , the licensing inspector observed and reviewed head count forms in the three year classroom. The center did not comply with 1:10 ratio. On February 23, 2018, one staff with eleven children at 7:53am- 8:00am. One staff with twelve children form 8:00am-8:04am. A second staff member arrived at 8:04 am. The three year old classroom was not in ratio for eleven minutes. In the four and five year old classroom, one staff with eleven children from 7:55-8:05. In the four and five year old classroom was not in ratio for 10 minutes. Staff interviewed confirmed the center is not complying with ratio.
