Technical Assistance:
Discussed with the Director: Medication documentation, Accident Reports and documentation of how parents are notified and time. Also discussed that no non enrolled children may be present.

Comments:
An unannounced monitoring inspection was conducted beginning at 6:00 and ending at 8:00. There were 17 children present, ages 5 - 11 with three staff. The Children participating in Zone activities including practicing for a play and physical activity. Reviewed staff and children's files - complete Reviewed medication - see violation notice Emergency drills are current as is the Health Inspection and Insurane. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on Observation, it was determined that there was one child present who was not enrolled at this SACC site and who did not have a file on site. Evidence: The child of Staff C was present with mother and another staff at 6:00. A second staff arrived shortly after that and Staff C took the child to his SACC site.

Plan of Correction: The Staff person had an emergency and needed to bring her child. In the future, staff will notify another staff to cover for her.

Standard #: 22VAC40-185-260-A
Description: Based on observation and interview with the Director it was determined that there is not a current Fire Inspection on file. Evidence: The last Fire Inspection report is dated 3/7/16. Inspections are due every year.

Plan of Correction: Director will discuss this with the Area Manager.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medications for 6 children, it was determined that not all medications have complete documentation. Evidence: The medications for Child #1 and #2 do not have prescription labels. The medication for Child #3 is not in the medication box although permission forms are present.

Plan of Correction: The Director will work with her area manager to get medications in order.

Standard #: 22VAC40-185-510-J
Description: Based on a review of medications and interview with the Director, it was determined that not all medications are locked in medication bag. Evidence: The medication for child #3 is not in the medication box. The Director stated that the child self carries his medication in his backpack which is accessible to all SACC children.

Plan of Correction: All medications will be locked.
