Today an unannounced monitoring inspection was conducted between the hours of 2:15pm and 4:15pm. I observed 36 children directly supervised by 2 staff. Ratios and supervision were in compliance. Children were observed arriving to the program, playing outdoors, doing crafts, playing with legos, doing homework, and eating snack. Today's snack consisted of milk and Cheerios. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Standard #: 22VAC40-185-550-L Description: Based on documentation review, injury reports did not have complete documentation of required information. Evidence: One of four injury reports was missing the date of parent notification and a second signature of either a parent or another staff member. Two of four injury reports were missing the date of parent notification. Three of four injury reports were missing the time of the incident, and all four injury reports reviewed were missing actions to be taken to prevent future incidents. Plan of Correction: We will complete the forms.
