- Technical Assistance:
Consultation was given on proposed CDC standards, medication authorization forms and out of state CPS checks for staff.
An unannounced renewal study was conducted today from 10:00-11:30 on-site and a separate visit was conducted at the Parks and Recreation office to review staff files as they are maintained at a central location. Fifteen children were present with two staff which is within ratios for supervision. A special visitor discussing nutrition and serving beans and rice and green tea ice cream was present. Emergency preparation was current for trainings, evacuation drills and first aid kits. Four emergency medications were on site and the Center Director has current Medication Administration Training (MAT) certification. Children transitioned to the rest room, hand washing and outside play on the site playground. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-510-C Description: Based on medication review, a written medication authorization has expired. Evidence: A long term medication for child #1 is past one year old. The parent signature expired 11/30/2017 and the physician signature expired 12/08/2017. Plan of Correction: Center Director will request the parent and physician complete a new written authorization for the medication.
Standard #: 22VAC40-185-550-M Description: Based on record review, one of one injury form is incomplete. Evidence: An injury form did not include "future action to prevent reoccurrence", the time that parent was notified, how the parent was notified and a second signature of either a parent or staff member. Plan of Correction: Injury form for center record will be completed with required information.
Standard #: 63.2-1721.1-B-3 Description: Based on record review, 2 of 3 staff members did not have the required out of state Central Registy Search (CPS) as required by 9/2017. Evidence: Staff #1 and Staff #2 are missing the out of state CPS check for a residence in another state within the last 5 years other than Virginia. Plan of Correction: Checks for staff who lived out of state within the last 5 years will be obtained as soon as possible.
