Today an unannounced Monitoring Inspection took place between the hours of 11:30am and 1:15pm. At the start of the inspection there were a total of 13 children with 3 staff; all ratios were incompliance. There were also 16 children in the kindergarten, first and second grade classrooms that are within the school. A total of 4 children?s files and 3 staff files were reviewed. The children were observed playing on the playground, washing their hands and eating lunch. The children bring in their own food from home. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain documentation of a physical exam for one child. Evidence: Child #1 record did not contain documentation of a physical exam. Plan of Correction: We have been asking the parents for this for a long time. We will tell them that we got a violation for this. We will discuss further measures with the family.
Standard #: 22VAC40-185-160-C Description: Based on record review, the center did not obtain results of a follow-up tuberculosis (TB) screening every two years for two employees. Evidence: Staff #1 most recent TB test results are dated 9/23/2015. Staff #3 most recent TB test results are dated 2/17/16. Plan of Correction: We will take actions.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff records did not contain all required information. Evidence: Staff #1 file did not contain documentation of two reference checks. Plan of Correction: We will ask her to provide two referenced from previous employees.
