Standard #: VENDSUB-000-006-B-11 Description: Based on a record review the center did not maintain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #1 (dob 12/05/14) had an allergy to milk and berries documented on the enrollment form and parent page of the school entrance form in addition to the center's "Allergy & Dietary Preference" sheet. The center did not have a written allergy plan to address the two documented allergies. Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The center has a meeting with the parent of child #1 today. More clarification on the child's allergy will be discussed and documented. A written allergy plan will be requested from the parent for the child's record.
