Comments:
An unannounced monitoring inspection was conducted today. There were 69 children in care, with a total of 13 staff members assigned to their direct supervision. Children's records, staff background checks, staff training certificates, staff records, the physical plant, emergency preparedness items, children's nutrition, and children's medications were inspected today.

The children were observed during open center activity time. A large variety of activity choices were offered to the children, including: music and movement, making bird eggs and nests, "assembling" Humpty Dumpty art projects, assembling leprechaun traps, eating snack, engaging in quiet activities, and engaging in creative play activities.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #2 does not include documentation showing that a parent has reviewed the child's contact information within the past year. This enrollment document was signed by the parent but not dated.

Plan of Correction: I will ask the parent to date the form.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training records, it was determined that one staff person has not completed all required components of First Day Orientation Training. Evidence: Staff E has not completed "Recognizing Child Abuse and Neglect" training. This person has a documented start date of 11/16/17.

Plan of Correction: I will make sure she does the training today.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that two potentially hazardous substances were not stored in an appropriate manner. Evidence: 1. In Room 1, a bag of planting soil was open and on the floor in the classroom. This item is labeled "keep out of the reach of children." 2. In the Quiet Room, a container of table cleaning wipes was stored inside an unlocked upper level cabinet.

Plan of Correction: The bag of planting soil was immediately moved during the inspection. The wipes were also immediately moved during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observations, it was determined that the outlets on one electrical power strip were not covered. Evidence: In the Art Room, the Inspector observed one electrical power strip on the counter. The outlets on this power strip were not covered.

Plan of Correction: That is not our power strip, but it was immediately moved during the inspection.
