22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 8:30am-12:45pm. At the time of entrance, 81 children were in care with 13 staff members present. Children were observed getting ready to go to school, snack time, washing hands, playing with table toys, engaged in group activities and infant care. A selection of staff and children records, medications and other documents were reviewed. The center is clean and organized. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-140-A Description: Based on review, the center did not obtain documentation that each child had a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. Child A began at the center on 12/29/2017 and the center did not have a copy of the child's physical. Plan of Correction: The parent was called while the inspector was present. The director requested a copy and the parent stated they will bring it in on 3/21/2018.
Standard #: 22VAC40-185-270-A Description: Based on observation, inside equipment of the center were not maintained in a safe and operable condition. Evidence: 1. In the Pre-K classroom, the children's play kitchen sink had four missing parts which caused 1 larger hole and 3 smaller holes. The larger hole could cause a finger entrapment. 2. In the Two year old classroom, the children's play kitchen sink had a missing part which caused a screw to be sticking out that could entangle clothing or snag skin. Plan of Correction: I ordered two new kitchen sinks for the classrooms and removed the nail immediately.
Standard #: 63.2-1720.1-B-3 Description: Based on record review, the center did not obtain a copy of the results of the search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: 1. Staff A began employment on 02/22/2016 and does not have documentation that a central registry was sent out for the state the staff member resided in the past five years. Plan of Correction: I sent it today.
