A self-reported incident was received by licensing on 3/6/18. Information was gathered related to the incident, and interviews conducted as needed. There is one violation resulting from this incident. Thank you for your time and cooperation. If you need further assistance please contact me.
Standard #: 22VAC40-185-410-1 Description: Based on information obtained the facility failed to maintain compliance with this standard as required related to physical punishment. Evidence: The facility self reported an incident that occurred on 03/06/2018 where staff #1 smacked child #1, 9 years old and the child of staff #1, in the mouth prior to him getting on the public school bus that morning. Staff #1 admitted to this action, and child #1 also reported this action to be true. This action was also witnessed by other staff and children in care at the time. Staff #1 was the teacher for the school age class that morning, which included her child-child #1. The child reportedly had a blood blister, and some bleeding of the lip with broken skin as a result of the smack to the mouth. Plan of Correction: Once the facility learned of this incident the staff member was immediately placed on leave pending results of the investigation. During the time on leave, and shortly after the incident, the staff member resigned. All staff obtained a review on appropriate discipline and behavior guidance for all children. Also, reporting of suspected child abuse was reviewed at this same time.
