An unannounced renewal inspection was conducted with center director and center staff. There were 45 preschool children present with six staff actively supervising. The children were engaged in a variety of activities including art projects, dance exercise, and eating lunches prepared from home. Two staff files and three children's files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted on 4/6/2017. The last fire inspection was conducted on 5/1/2017. One child's emergency medication was reviewed and is in compliance. Standard #: 22VAC40-191-60-C-1 Description: Based on review of two staff files, the facility failed to have documentation of an original criminal history record report within 30 days of employment. Evidence: Staff B (date of hire 9/7/2017) did not have documentation of an original criminal history record on file. Plan of Correction: The original fingerprint card was sent to and reviewed by the Diocese; however, it was not sent to facility director for the file. Staff B will have a new fingerprint check conducted. Update **Completed and returned to director
Standard #: 22VAC40-191-60-C-2 Description: Based on review of two staff files, the facility failed to have documentation of a central registry report (CPS) within 30 days of employment. Evidence: Staff B (date of hire 9/7/2017) did not have documentation of a CPS check/central registry report on file. Plan of Correction: The central registry report was sent to and reviewed by the Diocese; however, it was not sent to facility director for the file. Center director will contact Diocese for the report and will conduct a new check if necessary.
