Thank you for your assistance during this unannounced inspection that was conducted on March 7, 2017. This inspection was initiated from a complaint that was received by the Valley Licensing Office that was receive on February 12, 2018 regarding supervision, staff qualifications and training, emergency procedures and medication. There were seventy-one children in the care of six staff. Six children's records and six staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. The information gathered during this complaint supports one of the allegations reported. The complaint is valid. Please complete a plan of correction and return to your licensing inspector within five business days. Description: Based on observation, the center failed to ensure ratios were being followed for children ages four through five. Evidence: 1. One staff member was supervising fourteen children, ages four through five, from 3:05 p.m. until 3:30 p.m. 2. Staff #4 and #5 confirmed only one staff member was supervising the pre-kindergarten and kindergarten group of children.

Plan of Correction: Moving forward, we will hire another staff to ensure that all ratios are met. There will be at least 2 staff with the 4 and 5 year old when over 10 children.
