22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted beginning at 3:50 p.m. and ending at 5:45 p.m. with the center director and three staff. There were forty-three children in care ranging in age from five to ten years of age. Children were observed having snack. Snack was Harvest Cheddar Snack Mix and 100% juice. After snack, children participated in large motor skills group game while part of the group went to computer lab with a staff member and the ECE teacher for homework time. After the group game, children were able to play at their choice of centers that included art, board games and reading. Five staff records and six children's records were reviewed. Four medications and documentation was reviewed. Fire drills were posted and up to date. The monthly snack menu was posted. Standard #: 22VAC40-185-130-A Description: Based on review of documentation, the center failed to obtain documentation of immunizations by the first day of attendance. Evidence: Child #3, enrollment paperwork completed 12/27/2017, did not have documentation of immunizations in the file. Plan of Correction: The Site Director requested the immunizations from the parents 3/12/18
Standard #: 22VAC40-185-140-A Description: Based on review of documentation, the center failed to obtain documentation of a physical exam within thirty days of enrollment. Evidence: Child #3, enrollment paperwork completed 12/27/2017, did not have documentation of a physical in the file. Plan of Correction: The Site Director requested the physical from the parents 3/12/18
Standard #: 22VAC40-185-160-A Description: Based on review of documentation, the center failed to obtain a Tuberculosis (TB) screening for staff within 21 days of employment. Evidence: Staff #4, hired 2/9/2018, did not have documentation of a TB screening in the file. Plan of Correction: A copy of the Staff member's TB test was requested from the Staff member again and she said she will bring it for her file the next day.
Standard #: 22VAC40-185-400-B Description: Based on observation, staff failed to provide behavioral guidance to children in care. Evidence: A school aged child was observed standing during snack and squirting his juice pouch into his mouth and all over his face. No staff intervened or provided redirection to the child's behavior. Plan of Correction: The Area Manager spoke with the new staff member who was subbing at the Site the same day to make sure she understands that we need to remind the children of appropriate behavior every time we witness inappropriate behavior.
Standard #: 22VAC40-185-500-A Description: Based on observation, children did not wash hands as required. Evidence: Children did not wash their hands after snack. Plan of Correction: The Area Manager spoke with the staff the same day to make sure they followed their schedule to wash hands after snack as well as before.
Standard #: 22VAC40-185-510-C Description: Based on review of documentation and medication, the center failed to follow it's procedures for medications. Evidence: Child #1's Epi Pen expired 12/2017 and was still stored in the medication box. Plan of Correction: The Site Director returned the medication to the parents immediately.
Standard #: 22VAC40-185-560-N Description: Based on observation, children were permitted to walk around while eating and drinking. Evidence: Three children were observed walking around eating and drinking during snack. Plan of Correction: The Area Manager spoke with the staff the same day to make sure everyone understands that we need to remind the children of appropriate behavior every time we witness inappropriate behavior
