63.2 Licensure and Registration Procedures
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on March 14, 2010 from 2:20 p.m. through 5:10 p.m. Five children's and five staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Areas observed were: arrival, snack, hand washing/bathroom procedures, free choice activity, homework and departure. Interactions between the children and staff were observed as positive. Standard #: 22VAC40-185-340-A Description: Based on observation, the center failed to ensure the care, protection and guidance when supervising children. Evidence: Two children were observed walking over two sofa's in the classroom Staff were unaware students had done this. Plan of Correction: Staff have been notified of the citation. A staff member will be with eye view of the sofa at all times. Children have been give the expectation to not walk over sofas.
Standard #: 22VAC40-185-500-A Description: Based on observation, center staff failed to ensure all children washed hands before and after eating snack. Evidence: Five children were observed not washing hands before eating snack. Three children were observed not washing hands after snack. Plan of Correction: A plan has been implemented that when children arrive off the bus and put their things away, they go directly to the bathroom to wash hands and sit down at assigned seats to eat snack. Staff members allow children to get up when done and go directly to washing hands after snack. A poster with these procedures has been created and put beside our daily schedule.
Standard #: 22VAC40-185-560-F Description: Based on observation, the center failed to list all snack items being served on the menu. Evidence: The menu for March 14, 2018 listed chips and salsa being served. In addition to chips and salsa, cheese sticks and oranges and gold fish crackers were offered. Based on observation, the center failed to comply with USDA requirements for snack being served. Evidence: Three children arrived on the late bus and were offered gold fish crackers. The center was out of cheese sticks and oranges, which were offered to the early group for snack. Plan of Correction: USDA guidelines have been posted on the refrigerator for staff. Staff have been reminded that two out of the four food choices must be offered. Director will ensure that enough fruit, dairy and grains will be available for late buses. The menu will be updated immediately by staff, if a change in snack has to be made.
Standard #: 22VAC40-185-560-K Description: Based on observation, center staff failed to sanitized the snack tables after children were finished eating snack. Evidence: Children were observed eating snack and continuing to play free choice games on the tables. The tables were not sanitized after snack before allowing the children to play free choice games on the tables. Plan of Correction: Staff have been notified that all children must finish snack and wash hands, before they go onto an activity. Activities at snack tables are no longer allowed during snack. Tables are sanitized after snack and then activities are allowed at those tables.
Standard #: 22VAC40-185-560-M Description: Based on observation, center staff failed to ensure all children were seated when eating snack. Evidence: Three children were observed walking in the classroom while eating snack. Plan of Correction: Staff and children have been notified that they must sit down and eat snack. This has been put on our poster that is placed beside our daily schedule.
Standard #: 63.2-1720.1-A Description: Based on record review, the center failed to ensure one employee had obtained eligible fingerprints before being employed. Evidence: Record review of Staff #1 did not have documentation of obtaining fingerprints before beginning work. Date of hire for Staff #1 was documented on 02/19/2018. Plan of Correction: Staff #1 completed fingerprints on 3/16/2018 at 11 am. Confirmation receipt forwarded via email to inspector. Results expected within 48 hours. Staff #1 has not been allowed to work since 3/14/2018
