22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today ins the Sprouts building. All groups are in ratio today. 3 staff and 5 children's records were reviewed. See violation notice for missing information. Emergency escape drills, medications, injury reports and first aid supplies are all in compliance. Children were observed in a variety of activities in all rooms today. The playground was not inspected today due to inclement weather. Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center were maintained in a safe and operable condition. Evidence: There is a hole in the wall in the gym approximately two feet off the ground that poses as a finger and hand entrapment. Plan of Correction: I will contact facility management to have the hole repaired.
Standard #: 22VAC40-191-60-C-2 Description: Based upon a review of staff records, one record did not contain a completed Central Registry Search (CPS) within 30 days of employment. Evidence: Staff A's (Date of Hire: 10/24/17) record contains a completed CPS search dates 12/13/18, past 30 days of employment. Plan of Correction: We had sent the form back for missed information. The completed form is now on file.
