An unannounced monitoring inspection was conducted on March 15, 2018 from approximately 12:50pm-2:45pm. There were 35 children present at arrival to the facility with five staff members. The children were observed being dismissed from the program in one classroom and going to the bathroom and napping in the second classroom. Children that were not resting were engaged in quiet activities on their cots. Three medications were reviewed in the facility on this date. Five children's records were reviewed. A large open field is being used for the outdoor play area, as the program is no longer utilizing the playground equipment at the school.
Standard #: 22VAC40-185-150-B Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's medical report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection conducted on March 15, 2018, the immunizations for Child #1 had not been signed by a physician, his designee, or an official of a local health department. Plan of Correction: When the parent picks the child up, the teacher will have the parent to have the form signed or stamped by the doctor.
Standard #: 22VAC40-185-60-A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each enrolled child's record was maintained with all required information. Evidence: During the inspection conducted on March 15, 2018, in the record for Child #1, there were incomplete addresses for two persons to contact in the event of an emergency where the parents cannot be reached. Plan of Correction: When the parent picks the child up, the teacher will have them to fill out the information.
Standard #: 22VAC40-185-440-E Description: Based on observation and inspection of the facility, the staff members did not ensure that there shall be at least 12 inches of space between occupied cots. Exception: Twelve inches of space are not required where cots are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage. Evidence: During the inspection conducted on March 15, 2018, four out of 17 occupied cots were observed with approximately 10 1/2-11 inches of space between them. Plan of Correction: The cots will be appropriately spaced during the next day naptime.
