32.1 Report by person other than physician
An unannounced inspection was conducted from 1:00pm-3:00pm. Upon arrival there were 72 children in care with 11 staff. A sample of 6 children and 3 staff records were reviewed. At the time of inspection there were no children with known food allergies and no medications. Children were observed during nap. Infants were observed in cribs, interacting with staff and being fed. Diapering and hand washing was also observed.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure areas and equipment of the center, inside and outside , were maintained in a safe condition. Evidence: There was peeling paint on the wall next to the back door, and on the one of the stairs of the composite play structure with the yellow slide. Plan of Correction: The following plan of correction was provided by the program director during the exit interview: The center's maintenance person stated the playground could not be painted in the winter. The director will contact maintenance again to have it all areas painted.
Standard #: 22VAC40-185-440-J Description: Based on observation the center did not meet all crib requirements. Evidence: In the infant room a baby was observed sleeping in a crib. The mattress had sides were curled up and there was more than 1 inch of space between the mattress and the crib. Plan of Correction: The following plan of correction was provided by the program director during the exit interview: The director will talk to infant staff about ensuring the mattress sheets fit the mattress and that they are flattened before placing a baby in a crib.
Standard #: 22VAC40-185-500-A Description: Based on observation the center did not ensure all requirements were met when hand washing. Evidence: In the infant room staff was observed changing a baby's diaper. The staff did not was her hands or the baby's hands after the diaper was changed. The staff put the child down and out on a pair of gloves, picked up the soiled diaper and opened the cabinet with the diaper in her hand before discarding it. Plan of Correction: The following plan of correction was provided by the program director during the exit interview: The director will retrain staff on hand washing and diapering.
