An unannounced Monitoring Inspection was conducted on March 21, 2018 from approximately 9:50 a.m. until 1:10 p.m. Upon entrance there were 21 children and 3 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 5 staff records. Children were observed during their chapel program. The center is no longer using the outdoor playground or administering any medications. Standard #: 22VAC40-185-60-A Description: Based on observation and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained all the required information. Evidence: During the inspection on March 21, 2018, 2 of the 5 records reviewed were determined to be incomplete. 1) The record for child #1 did not include the street name, house number, city, state and zip code for the second emergency contact listed. 2) The record for child #12 did not include the city, state and zip code for the second emergency contact listed. Plan of Correction: The center director will have the parents fill that in today,
Standard #: 22VAC40-185-550-A Description: Based on observation and staff interviews, the licensee did not ensure they had an emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and shelter-in-place. The plan, which shall be developed in consultation with local or state authorities, addresses the most likely to occur emergency scenario or scenarios, including but not limited to natural disaster, chemical spills, intruder, and terrorism specific to the locality. Evidence: During the inspection on March 21, 2018, the center program director confirmed the emergency plan available for review had not been reviewed by local or state authorities or developed with consultation from local or state authorities. Plan of Correction: The center director will the plan reviewed.
Standard #: 22VAC40-185-550-B Description: Based on observation and staff interviews, the licensee did not ensure they had an emergency preparedness plan that contained all the required procedural components. Evidence: During the inspection on March 21, 2018, the following information was not documented in the emergency plan that was available for review; 1) Establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each 2) Availability and primary use of communication tools 3) Plan for children's head counts and the secondary means of egress for evacuation of the building 4) Securing essential documents (sign-in records, parent contact information, etc.) and special health supplies to be carried into the designated assembly points 5) Staff training requirement, drill frequency, and plan review and update Plan of Correction: The center director will update the plan and the stuff to it.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and staff interviews, the licensee did not ensure the findings of the most recent inspection of the facility were posted in a conspicuous place for the public to view. Evidence: During the inspection on March 21, 2018, the most recent finding of the inspection conducted on July, 27, 2017 were not posted. The inspection findings posted were dated February 23, 2017. Plan of Correction: The center director will post today's inspection.
