Comments:
An unannounced monitoring inspection was conducted today, March 20, 2018 to determine compliance with licensing standards. The Inspector arrived at 2:30 pm and concluded the inspection at 4:00 pm. The facilities owner participated in the inspection. Upon the inspectors arrival, the children in the infant room were playing with a pop-up tunnel, wooden blocks and finishing snack at the table. Toddlers were observed finishing snack. Children provide their own meals which were labeled with names. As children finished snack hand washing was conducted before children started work cycles of their choice. Children worked with puzzles, blocks and played with dolls. Later children had time outside on the playground. Staff interacted in a positive manner. All areas of the facility were inspected to include two classrooms, hallway, bathroom and out door playground. Required information was posted to include: The License, last inspection notice, emergency number, evacuation maps/procedures and daily schedules. The facility has age and stage appropriate supplies and materials. . The first aid kit and non medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records (see violation notice) and five children's records were reviewed. Medication and medication authorizations were reviewed. Staff have current EMAT certification. Multiple staff have current CPR, First Aid and Daily Heath Observation training, The most recent evacuation drill was documented on 03/07/2018. The two required shelter-in-place drills for 2017 were conducted on 09/20/2017 and 12/13/2017. Two shelter-in-place drills for 2018 have also been completed, 02/06/2018 and 03/20/2018. Today's Ratios: Infants: 5 children with 2 staff. Toddler: 12 children with 3 staff. Standard #: 22VAC40-185-160-A
Description: Based on review of staff records, the facility did not obtain documentation of a tuberculosis screening for staff that had been completed within 12 months prior to or 21 days after employment. Evidence: Staff #4, hired 08/28/2017, had documentation of a tuberculosis screening dated 01/10/2016, exceeding 12 months prior to hire.

Plan of Correction: Per the Director: We will have staff update her tuberculosis screening and check with new hires that screening are completed within the 12 months prior to hire or 21 days after hire.
