22VAC40-191 Background Checks (22VAC40-191)
Licensing Inspector arrived at 11:00am and departed at 1:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 52 children in care with 14 staff present. Children were observed a sample of 2 staff and 5 children's records were reviewed in addition to 4 medications and 1 over-the-counter skin product. Children were observed eating lunch, during nap and being read a story. Infants were observed sleeping in cribs and being fed by staff.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure areas and equipment of the center, inside and outside, were be maintained in a safe condition. Evidence: 1. In the gym there was peeling paint on the wall in various places in reach of children. 2. In the twos classroom there was a hole in the wall in reach of the children that measure about 1/2 in inch. The dry wall around the hole was cracked and peeling. 3. The hardware on the toddler swing set was rusted. Plan of Correction: This will be corrected by 4/19/18. Staff will be retrained by 3/23/18 to ensure they are completing their classroom checklists and reporting any deficiencies to management team within 24 hours. Management will inspect periodically for compliance. DM will inspect on visits.
Standard #: 22VAC40-185-330-B Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: The toddler/twos playground had 4 inches of pea gravel in the fall zones of the swing set and slide. Six inches is required. Plan of Correction: This will be corrected by 4/19/18 Our AM teacher will complete our playground checklist daily before children go on the playground. Any deficient items will be reported to management within 24 hours. Our safety captain and management team will do weekly safety checks to ensure compliance. DM will inspect upon periodic visits.
Standard #: 22VAC40-185-440-B Description: Based on observation, interview and a record review the center did not meet the requirements when using cribs. Evidence: Child #6 (dob 11/20/16) was observed sleeping in a crib that was labeled with child #4's name. The staff member stated child #6 was "just visiting," for the day. Staff reported child #6 is normally in the toddler room. Plan of Correction: Staff will be retrained by 3/23/18 with proper cot and naptime procedures. Cots will be available for naptime for the older children effective immediately. Management will ensure compliance through periodic inspection.
Standard #: 22VAC40-185-550-D Description: Based on a record review the center did not have documentation that monthly evacuation drills were practiced. Evidence: The center's last documented fire drill was dated in October 2017. Plan of Correction: This will be corrected by 3/23/2018. Our safety captain will schedule required monthly evacuation drills to ensure compliance. Drills will be recorded by management team in the Emergency Drill Log with supporting documentation. Management will ensure compliance through periodic inspection. DM will inspect upon visits.
