Licensing inspector arrived at 9:40 am and departed at 12:00pm to conduct a licensing inspection and a Subsidy Health and Safety -inspection. Upon arrival there were twenty -two children in care with six staff. Additional staff were present to assist. Children were observed working on spelling thier names, writing words, story time, and station activities . The sample size consisted of 5 children records, 5 staff records and 3 injury records. Areas of non-compliance was reviewed with the director.
Standard #: 22VAC40-185-60-A Description: Based on 2 of 5 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on March 22, 2018, the following record did not contain all required documentation: Child #1 did not have work phone number. Child #2 did not have a complete address for emergency contacts. Plan of Correction: The program director will get information from parent.
