Comments:
A Subsidy Health and Safety inspection was conducted on 3/16/2018 to review supplemental health and safety requirements. 42 children were in the care of 9 staff present. Children were observed during arts and crafts, toileting and hanwashing, transitions to lunch and naptime. A sample of 5 staff records, 5 children's records, emergency drills and medications were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please contact me if you have any questions at Ivaco.Clarke@dss.virginia.gov or 571-835-5505.

Violations:
Standard #: VENDSUB-000-005-B
Description: Based on a review of records, the written attendance did not document the arrival and departure of children as it occurred. Evidence: Upon inspector arrival, 15 children were present in classroom A; however 15 children were noted on the written roster. The written roster for 45 children receiving subsidy payment did not document their arrival and departure on March 14th - 15th or the arrival for the 45 children on the day of inspection [March 16].

Plan of Correction: The roster for Classroom A was corrected during the inspection. We will implement a new attendance roster that will allow teachers to document all children's arrival and departures.
