- Technical Assistance:
Discussed with center management and staff the need for additional training on recognizing and reporting suspected child abuse and neglect. Discussed obtaining permission from parents prior to any field trips or activities outside the facility.
An unannounced monitoring inspection was conducted on 3/13/18 triggered by a self reported incident of a staff member rough handling a child and forcing the child to assume an uncomfortable position. Staff were interviewed. Based on the preponderance of evidence, violations were cited related to the incident. A full monitoring inspection was conducted on this date as well. During the inspection, there were 51 children in direct care of 11 staff members. Children were observed getting fed their bottles, doing a craft activity, enjoying circle time and singing songs. The Pre-K classroom was preparing for a fancy luncheon. The Pre-K children had the opportunity to dress up for the fancy luncheon. 5 staff/children's records were reviewed. Standard #: 22VAC40-185-400-B Description: Based on staff interviews and record review, staff did not use behavioral guidance that was age and stage appropriate with the intent to redirect children to appropriate behavior. Evidence: 1. On 3/8/18, Staff # 1 and Staff # 2 took a a group of 13 children to the building's 11th floor instead of going to the playground due to cold weather. 2. Staff # 2 stated verbally and in written statement that she had 8 out of the 13 children with her. Staff # 2 stated that the rest of the group was with Staff # 1. 3. When Staff # 2 and her group of children sat down by one of the windows, she looked over at the other group and saw Staff # 1 holding child # 1 upside down by one foot. Staff # 2 stated that child # 1 was off the ground as Staff # 1 was holding the child upside down. Staff # 2 stated that Child # 1 was upset and crying, and the child's shoes were off. 4. Staff # 2 stated that Staff # 1 held the child in that position until the child calmed down. 5. Although Staff # 1 denies holding the child upside down, she acknowledged that she had to redirect Child # 1 by holding her around the feet and ankles as the child was kicking Staff # 1 during her temper tantrum. Plan of Correction: Staff # 1 was terminated effective 3/9/18. All staff were retrained on mandated reporting requirements verbally and in writing.

Staff # 3 stated that she talked to Staff # 5 about the incident. Plan of Correction: Staff # 1 was terminated effective 3/9/18.
Standard #: 22VAC40-185-580-L Description: Based on record review and staff interview, parental permission for field trips was not secured by the center before the scheduled activity. Evidence: 1. On 3/8/18, a group of 2-3 year old children were taken on a walking field trip to the 11th floor of the building. 2. The center did not secure parental permission from the parents before the scheduled activity. 3. Staff interviews have indicated that the children were being taken to the 11th floor frequently as an alternative to the playground due to cold weather. Plan of Correction: All staff have been told not to use unoccupied floors for times of inclement weather, keep us inside. A blanket permission form has been added to the child registration form.
