Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:00pm. At the time of inspection, 60 children (two years through five years) were directly supervised by 10 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed creating art to learn about India and the culture, engaged in group pretend play, and making crafts to celebrate Easter. Staff were positively engaged with the children in many activities. There was an abundant supply of books, toys, and materials for the children. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, potentially hazardous substances such as cleaning agents were not kept in a locked place. Evidence: In the three year old classroom, a cabinet that stored cleaning agents did not have a working lock.

Plan of Correction: The lock was repaired at the time of inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, the center did not have an original central registry finding within 30 days of employment of a staff member. Evidence: The record for staff #2 (start date: August 2017) did not have a completed central registry finding. The form was returned to the center incomplete.

Plan of Correction: The form will be resent for completion. The staff member has completed the criminal history check and fingerprinting.
