Discussed CPS checks for people living outside of the State within the last five years.

A complaint investigation was conducted at 10 AM. The Inspector interviewed staff, observed two classes, reviewed staff records and attendance records in response to an allegation that the infant room is over ratio. The Inspector did not have preponderance of evidence to support the allegation and it has been deemed not valid. The Inspector did find violations during the course of the investigation that were not part of the original complaint and the findings have been disclosed in this report. Description: Based on record review, it was determined the center did not verify written information to demonstrate that Staff Member #2 possesses the education, and experience required by the job position. Evidence: 1. Staff #2 was hired to be a lead in a classroom. The file for the staff member was reviewed and it contained the resume stating education was completed and that the staff member had over 6 months experience in a child care facility. 2. The file did not include documentation to verify the staff member's education as there was not a copy of a diploma or transcript. The file also did not include documentation that the child care facility was contacted to confirm listed experience.

Plan of Correction: We will verify all staff and document this in each file.

Description: Based on observation and staff interviews, it was determined the center was using a piece of equipment that was not maintained in a safe and operable condition. Evidence: 1. Inspector was in the infant room and observed a crib that was different from the other cribs. Upon review of the crib, it was observed that a bolt on one side was loose and the base was not secure. The crib was an evacuation style crib but had no compliance or manufactures date attached to the equipment. 2. Staff were asked how old was the crib and a staff member, who have been employed over 10 years, said the crib has been here as long as they have been there. 3. The remaining cribs had Compliance stamps or dates to verify it was manufactured after 2011. 4. Cribs that were manufactured before 2011 had to have paperwork to verify it met with Federal Regulations for safety and this crib did not have this.

Plan of Correction: We will remove it today as we were unaware this crib did not meet the standards.

Complaint related: No
Description: Based on observation, it was determined the infant classes did not have a nonabsorbent surface for diapering or changing infants. Evidence: 1. The Inspector observed the equipment and materials available to the infants and saw both changing pads used had rips and small holes which resulted in exposing the absorbent material inside the pad.

Plan of Correction: We have ordered new pads.
