Comments:
Today an unannounced renewal study was conducted beginning at 9:15am . There were 70 children present today with 14 staff. Children were observed being dropped off by their parents, working on arts and crafts and playing in the gymnasium. Children?s records, medications and emergency drills were reviewed. Staff records are reviewed annually in the administrative office. The inspection, including exit interview, ended at 12:00pm. Standard #: 22VAC40-185-140-A
Description: Based on record review, one child did not have a physical exam. Evidence: Child #3's file did not contain documentation of a physical exam.

Plan of Correction: I'll talk to the parents tonight and have them provide me with a physical.

Standard #: 22VAC40-185-40-D
Description: Based on observation and interview, the center did not have their license posted in a place conspicuous to the public. Evidence: While reviewing the centers required postings, the center's license was not posted. The programs director was able to locate the license that had fallen and been stored away.

Plan of Correction: This was corrected during the inspection.
