Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Tuesday, March 20, 2018 from 10:30am to 1:00pm. The children were observed playing in learning centers and getting ready for lunch. Lunch consisted of chilli with corn, mini cornbread, pineapples and milk. A menu was posted. The infants were observed being fed and sleeping. All classrooms inspected today. Due to the rainy weather the facility?s mini buses and playgrounds were not inspected. Ten children?s records and Ten employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Total enrollment today: 105 Today, the following child to staff ratios were observed: Infant I ?8:3 Infant II- 7:2 Toddler I ?10:2 Toddler II-5:1 Younger Twos ? 16:2 Older Twos ? 14:2 Threes ?25:3 Pre-K-20:3 School Age ?not present The violations from the previous inspections were checked for corrections. Description: Based on a review of children's records on 03/20/2018, the center did not ensure that two of ten records reviewed contained documentation of a physical examination before the child's first day of attendance or within one month after attendance. EVIDENCE: 1. The record of Child #1, who started at the center on 12/26/2017, did not contain documentation of a physical examination 2. The record of Child #2, who started at the center on 08/27/2015 did not contain documentation of a physical examination

Plan of Correction: Contacted parents and will ask them to bring in tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 03/27/2018, the center did not ensure that one of ten records reviewed contained the address of two designated people to call in an emergency if a parent cannot be reached. EVIDENCE: The record of Child #1 did not have an address listed for both of the emergency contacts listed.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 03/20/2018, the center did not ensure that all hazardous substances such as cleaning products were kept in a locked place. EVIDENCE: Bottles of cleaning supplies was observed on top of an unlocked cabinet in the Older Two's room.

Plan of Correction: The assistant director went to get a lock for the cabinet.

Standard #: 63.2-1720.1-B-3
Description: Based upon a review of staff records on 03/20/2018, the center did not ensure that one of ten records reviewed contained the required out of state child registry background check searches. EVIDENCE: Staff #7, whose date of hire is 08/29/2017, indicated on the Virginia Central Registry that she had lived in a state outside of the state of Virginia within the past 5 years. The record of Staff #7 did not contain the results of a central registry search for the other state.
