Comments:
An unannounced renewal inspection was conducted today March 22, 2018 to determine the facility's compliance with licensing standards. The inspection was initiated at 1:15 pm and concluded at 5:30 pm. Upon the inspector's arrival children were napping. After nap observations were made with children engaged in a variety of activities. Infant children were observed playing on the mats with a pop-up tunnel, balls and stuffed toys. Infants were being fed at the table and some were being held while drinking a bottle. Staff read stories, rocked and interacted on the floor with children in a positive manner. Staff were observed sanitizing toys and showed systems are in place for daily cleaning. Toddlers (Meadow) were at the tables for snack. After snack children were returning to time working puzzles. The two year old class (Forest) had time outside. children in the preschool classrooms participated in center activities that included magnetic story boards, blocks, play dough, legos, doll house, office (phones, keyboards, paper and pencils), gardening (children make flower arrangements with pots and artificial flowers), writing, and sorting (sorted farm animals by color, animal, home). Children told the inspector about their block creations, one child had made dinosaur feet and shared information about dinosaurs. Preschoolers have individual progress report notebooks with samples of class work that parents can review. In the Avenues building school age children were observed drawing and playing games in the art room, working puzzles in 64 West classroom, making seashells and other ocean creatures out of paper for the window mural in the library, and playing basketball in the Rapids gym. School-ager are continuing a theme of around the word and incorporating it into daily activities. Children named cities in Australia while shooting hoops with basketballs. Japanese receipts had been made in the cafe and Australian pockets had been made with pottery in the art class. Today's lunch consisted of a vegetarian wrap (provolone, lettuce, tomato, cucumber) with honey mustard dressing, tomato soup, peaches and milk. All areas of the facility were inspected to include both buildings and playgrounds. . The facility has age appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected for both buildings and all floors. Staff interactions with children were positive. The following information was posted: the License, menu, daily schedule, emergency numbers and various parent information. The inspector interacted with staff and children in care. All classrooms were within the required staff to child ratio. Fifteen children's records and eleven staff records were reviewed. Medications and authorizations were reviewed. Staff have current CPR, First aid and daily health observation training. Multiple staff have MAT certification. The most recent evacuation drill was documented on 02/07/2018. The two required shelter-in-place drill for 207 were completed on 02/21/2017 and 11/21/2017. Fire inspection: 09/07/2017, Health/ Pool Inspection 07/18/2017 Violations cited during the previous inspection were checked for corrections. There were no repeat violations. Description: Based on review of records, the facility did not obtain a criminal history record check for the licensee before three years since the dates of the most recent criminal history record check report. Evidence: The most recent criminal history check for the licensee was dated 03/28/2014, exceeding the three year allowance to renew.

Plan of Correction: Per the administrator: The licensee agrees to submit forms in a timely fashion. New forms have been filed.

Standard #: 63.2-1720.1-B-3
Description: Based on review of staff records, the facility did not obtain a copy of the results of a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years from hire. Evidence: 1. Documentation indicated that staff #2 lived outside the state of Virginia within the past five years prior to hire. 2. Staff # 2, hired 02/13/2018, did not have an out of state abuse and neglect search request by the end of the 30th day of employment.

Plan of Correction: Per the administrator: Now that we are aware of the new regulation, we will follow this procedure. * Please note we were unaware of this change in procedure.
