An announced record review was conducted on March 29, 2018. All of the three staff records for the center were reviewed. Areas of noncompliance are identified on the violation notice. The results of the inspection and violations were reviewed with the Recreation Program Coordinator.

Standard #: 22VAC40-191-60-B
Description: Based on record review, in one of three staff records reviewed, the licensee did not ensure that an employee of a licensed child welfare agency must not be employed until the agency has the person's completed sworn statement or affirmation. Evidence: During the inspection conducted on March 29, 2018, the following staff hired after July 1, 2017, completed the outdated version of the sworn statement or affirmation: Staff #1-hire date 11/13/17

Plan of Correction: All staff will complete the correct version of the sworn statements or affirmations.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, in one of three staff records reviewed, the licensee did not obtain a copy of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: During the inspection conducted on March 29, 2018, the record for Staff #2 (hire date11/21/15) was reviewed and documentation indicated that the staff member resided outside the state of Virginia within the last five years. The center had not obtained an Out of State Child Abuse and Neglect Search Request.

Plan of Correction: Will contact the state for potential results.
