An unannounced monitoring inspection was conducted on March 27, 2017 from approximately 8:45 am until 1:10 pm. At the time of entrance there were 54 children and 13 staff members present with children. There were additional staff on site for kitchen and administrative purposes. The sample size consisted of 6 children's records and 6 staff records. One medication was reviewed. Children were observed during center time, to include blocks, video games, puzzles and dress up. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center program director during the exit interview. Please contact the licensing inspector with any question at 404-0568.
Standard #: 22VAC40-185-60-A Description: Based on a review of 6 children's records and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled with all required information. Evidence: During the inspection on March 27, 2018, the following records were determined incomplete; 1) The record for child #2 did not include the home number for both parents listed. 2) The records for child #3 and #4 did not include the employment phone number for one parent listed. Plan of Correction: The center director will get the numbers today.
Standard #: 22VAC40-185-500-B Description: Based on observation and staff interviews, the licensee did not ensure there was a leakproof covered receptacle for soiled linens. Evidence: During the inspection on March 27, 2018, there was not a receptacle for soiled linens in the toddler room (room #109). The two's/three's room (room #108) and the transitional toddlers (room #106) had containers for soiled linens but the containers were not covered. Plan of Correction: The program director fixed it today during the inspection.
Standard #: 22VAC40-185-510-J Description: Based on observation and staff interviews, the licensee did not ensure that all medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on March 27, 2018, in the two's room (room #109) one medication epinephrine was observed in a plastic container on the bottom shelf of an unlocked cabinet. The bottom shelf was accessible to the children. Plan of Correction: The center director will ensure through staff training that all medication will be in the locked cabinet.
Standard #: 22VAC40-185-570-E Description: Based on observation and staff interviews, the licensee did not ensure prepared infant formula was refrigerated, dated and labeled with the child's name. Evidence: During the inspection on March 27, 2018, 1 out of 4 bottles observed in the refrigerator were not labeled or dated with a child's name. Plan of Correction: The center director will retrain the staff today.
Standard #: 22VAC40-185-570-H Description: Based on observation and staff interviews, the licensee did not ensure that prepared baby food was discarded or returned to the parent at the end of each day. Evidence: During the inspection on March 27, 2018, in the refrigerator in the infant room refrigerator, 3 out of 4 bottles of formula were observed in the refrigerator and were labeled for child #1 and were dated for yesterday (3-26-2018) and had not been returned home to the parent. Plan of Correction: The center director will retrain the staff and send home the bottles today.
Standard #: 22VAC40-191-60-B Description: Based on documentation review and staff interviews, the licensee did not ensure all staff had completed a sworn statement or affirmation before employment begins. Evidence; During the inspection on March 27, 2018, 3 out of 6 of the records reviewed did not include the most recent sworn statement with the question have you lived outside of Virginia in the past 5 years?. The first date of employment for staff #1 was 12-4-17, staff #3 was 11-17-17 and staff #4 was 11-17-17. Plan of Correction: The center director will obtain the updated sworn statement today.
Standard #: 63.2-1720.1-B-3 Description: Based on documentation review and staff interviews, the licensee did not ensure all staff that had lived outside the state of Virginia within the last five years had obtained a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence; During the inspection on March 27, 2018, 1 out of 6 staff records reviewed included documentation that staff #2 had lived outside of the state of Virginia within the last 5 years. The record for staff #2 did not include the required Out of State Search of the Central Registry Check. The first date of employment for staff #2 was 10-09-17. Plan of Correction: The center director will obtain the background check today.
