Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out. Discussed with center management making sure that fire exits are not blocked, overall cleanliness of the facility, lunch time.
An unannounced renewal inspection was conducted between 9:45am to 1:15pm. There were 28 children in care of 7 staff members. The children were observed playing in different centers, departing for and returning from a field trip, playing with play dough, eating lunch and getting ready for nap time. The lunch menu included chicken patties, green beans, carrots, applesauce and milk. 5 children/staff records were reviewed. Emergency drill records and injury reports were reviewed. There were no medications on site. Areas of non - compliance are discussed in the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, areas inside the center were not maintained in a clean, safe and operable condition. Evidence: 1. Peeling paint was found on the walls by the center's storage rooms in the multi purpose room. 2. Peeling paint was found in the 2's classroom by the back exit. 3. Areas of the flooring around the center showed dirt and dust that has settled over time. There was visible dirt on the area rugs in the classrooms. Dirt was also observed under and behind the interior furnishings of the center. 4. Inside play equipment being used by the children did not appear to have been cleaned. 5. The diapering table in the toddler room had dirt under the changing pad indicating that the surface had not been cleaned or wiped down. Plan of Correction: Not available online. Standard #: 22VAC40-185-520-A Description: Based on observation, over the counter products were kept past their expiration date. Evidence: The tube of antibacterial ointment in the first aid kit had expired on 9/2015. Plan of Correction: The tube was removed during the inspection.
Standard #: 22VAC40-185-560-N Description: Based on observation, children were walking around as they were eating. Evidence: 1. In the toddler room, two children were observed walking around, eating from their plates while the teacher was putting the mats down for nap time. Plan of Correction: Not available online.
