Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on March 29, 2018 from from 11:15 a.m. through 2:00 p.m. Upon arrival, there were seventeen children in the care of three staff. Five children's and five staff records were reviewed and updated. Areas observed were indoor play, hand washing/bathroom procedures, lunch, and a short movie. Interactions between the staff and children were observed as positive and the children seem comfortable with their daily routine and schedule. Standard #: 22VAC40-185-160-A Description: Based on record review, the center failed to obtain a current tuberculosis screening for one employee within 21 days of hire. Evidence: 1. Five staff records were reviewed. 2. Staff #3 submitted a tuberculosis screening that was dated 05/15/2015. 3. The date of hire for Staff #3 was documented on 01/04/2018. 5. Staff #6 confirmed the tuberculosis screening had not been completed within 12 months of Staff #3's date of hire. Plan of Correction: Staff #3 will obtain a tuberculosis screening as soon as possible.
Standard #: 22VAC40-191-60-B Description: Based on record review, the center failed to obtain a sworn statement or affirmation for one new employee. Evidence: 1. Five staff records were reviewed. 2. Staff # 3 did not have documentation of a sworn statement or affirmation. 3. The date of hire for Staff #3 was documented on 01/04/2018. Plan of Correction: Staff #3 will complete a sworn statement when she returns to work on Monday, 04/02/2018.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center failed to obtain a completed central registry finding for within 30 days of employment for one employee. Evidence: 1. Five staff records were reviewed. 2. Staff #3's record contained a central registry finding, however, it had not been returned because it had not been accurately completed for processing. 3. Staff #6 confirmed the central registry finding had not been processed. 4. The date of hire for Staff #3 was documented on 01/04/2018. Plan of Correction: A new central registry search will be correctly completed for Staff #3 when she returns on Monday, 04/02/2018. Documentation will be sent to the licensing inspector.
