22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 11:00am-12:30pm. At the time of inspection, 84 children (infants through eight years) were directly supervised by 15 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed during lunch time (chicken patty, peas, fruit, and milk), engaged in group play, and creating art projects. Diapering and handwashing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C Description: Based on documentation review, the center did not obtain updated tuberculosis (TB) tests or screenings for each staff member every two years. Evidence: The records for staff #1 and #4 did not have updated tuberculosis tests. Plan of Correction: The staff will go to have updated TB tests.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain equipment as required. Evidence: In the Intermediate classroom, a blue couch for the children had a large hole in side. Plan of Correction: We will dispose of the blue couch.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers. Evidence: In the Pre-Kindergarten classroom, an electric power strip did not have protective covers in the unused outlets. Plan of Correction: I will put protective covers on today.
