Standard #: 22VAC40-185-330-B
Description: Based on observation the center did not ensure where playground equipment was provided, resilient surfacing complied with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 . Evidence: On the playground the poured-in-place surfacing underneath the plastic composite play structure had 2 gaps in the surfacing measuring about a foot each in length.

Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The Assistant director will contact maintenance to have it repaired within the next few days.

Description: Based on observation the center did not follow all requirements when choosing to provide food and snacks. Evidence: There was not a menu posted in the front lobby where the parents check children in or other location conspicuous to parents

Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The Assistant Director will post the menu in the front lobby today.
