A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. Licensing Inspector arrived at 10:45am and departed at 1:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 71 children in care with 13 staff present. A sample of 2 staff and 8 children's records were reviewed. Children were observed during free play, eating lunch and during nap. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
Standard #: VENDSUB-000-006-B-11 Description: Based on a record review the vendor did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #7 and #8 were listed on the vendor's "Allergy & Dietary Preference," document as having food allergies. Neither child had a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Plan of Correction: The following corrective action was provided by the Assistant Director during the exit interview: Neither of these children have medications on site. The director will talk to the parents of both children and obtain a written allergy plan for both.
Standard #: VENDSUB-000-012-B Description: Based on a record review and interview with the Assistant Director, the vendor did not have documentation that staff completed the Virginia Preservice Training for Child Care Staff. Evidence: The center was unable to locate documentation that staff had completed the required Virginia Preservice Training for Child Care Staff. Plan of Correction: The following corrective action was provided by the Assistant Director during the exit interview: All staff have the preservice training. The center was flooded back in January and all records had been moved. The director will have each staff log into their accounts and print out the trainings.
